Care Alliance Health Center’s outreach and community engagement team helps connect the community to critical health care services and everyday living essentials. Our outreach program has played a key role in our success in serving under-served populations. Care Alliance’s outreach efforts allow us to reach individuals experiencing homelessness.
In partnership with local safety-net agencies we provide health care in encampments and permanent supportive housing. Our outreach team works extensively to build trust and link individuals to care.
Homeless Resource Card
The Northeast Ohio Coalition for the Homeless (NEOCH) publishes a Street Card annually. The Street Card was created to provide vital resources to individuals experiencing homelessness in our community.
